Advance Care Services provides a range of NDIS services to support daily living, personal care, community participation, independent living and higher support needs. Our services can be tailored around your NDIS plan, personal goals and preferred routine.

Core Support

Core Support helps participants with daily activities, personal routines, community access, transport, social participation and household tasks. This support is designed to make everyday life easier and more manageable.

Support Coordination

Our Support Coordination service helps participants understand their NDIS plan, connect with providers, build capacity and make better use of funded supports.

Supported Independent Living

Supported Independent Living is for participants who need daily support while living in a shared or individual supported home environment. We provide help with routines, personal care, household tasks and skill building.

High Intensity Support

High Intensity Support is available for participants with more specific care needs. This may include complex bowel care, catheter care, wound care, subcutaneous injections, PEG feeding support and other approved supports.

Personal Care

Our Personal Care support helps participants with daily routines such as showering, dressing, grooming, hygiene, meal routines and mobility. Support is provided with respect, privacy and dignity.

Community Access

Community Access support helps participants attend appointments, go shopping, join social activities, visit family and take part in local community life.

NDIS Referral

You can make a referral for yourself, a family member, a friend or a participant you support. Our team will review the referral and contact you to discuss the next steps.
