Anyone can make a referral to Advance Care Services. You can refer yourself, a family member, a friend or a participant you support. We welcome referrals from participants, families, carers, Support Coordinators, health professionals and other members of a participant’s support network.
To help us understand the participant’s needs, it is helpful to provide basic information such as contact details, NDIS plan information, preferred services, support goals, location and any relevant health or support information. If you do not have all of this information, you can still submit a referral and our team will assist you.
Yes. High Intensity Support is designed for participants who require specialised assistance with complex daily care and health-related support tasks. Supports are delivered in accordance with the participant’s NDIS plan, care plans and relevant clinical or health professional recommendations.
Yes. Advance Care Services can provide High Intensity Supports within the participant’s home, including Supported Independent Living, private residences and other approved environments, depending on individual needs and service availability.
